NEW Mesa Safety Forms App

With the help of the UX department, the Safety team is excited to introduce the new and redesigned Safety Forms App. 

 

This will replace the current forms app which was previously accessible through the wellness checks link texted to company devices daily. 

 

The new Safety Forms App contains the links to your favorite work forms below! 

• Report an Incident 24/7/365 - Used to report a work-related injury (no matter how minor) and connect with a telephonic nurse who will triage the injury and connect you to a medical professional if needed. 

• Pre-Task Safety Plan - Form used for recognizing hazards, assessing potential consequences of hazards, and implementing controls to eliminate or reduce hazards faced daily by our field teams. 

• Supervisor Site Inspection - Form used to document safety inspections of work conducted by Mesa employees or Mesa subcontractors.

• Employee and Supervisor Incident Reports - Forms used to document injuries, near misses, STOP WORK authority actions, and more. 

• Vehicle Accident Kit and Vehicle Inspection App - Apps used to document vehicle accidents and required monthly inspections of drivers.

• SDS Site Search - Site used to search for Safety Data Sheets of chemicals we use.

• Field S/H Handbook - A link to a copy of the Mesa Field Safety & Health Handbook. 

• Mesa/EMCOR IIPP - A link to a copy of the Mesa/EMCOR Injury and Illness Prevention Plan.

• Heat Illness Prevention Program - A link to a copy of the Mesa Heat Illness Prevention Program.

• Mesa/EBS Safe Work Practices - A link to a copy of the Mesa/EBS Safe Work Practices. 

• Warehouse Inspection App - App used to conduct daily and weekly warehouse inspections including the oil reclamation weekly inspection. 

• Where's the Fault Challenge - Form to complete a Where's the Fault learning submission and a chance to win Milwaukee gear!
